Chen is stressed: he's overworked and sleep deprived as his obstetrician wife Ming monitors her fertility with military precision. Fitz, who is working as an air ambulance doctor, arrives at Mercy.

Fitz and Ming talk about their insemination plans. Chen pursues his dalliance with Sienna, a flirtatious resident.

Chen confronts Fitz with an old stash of his letters to Ming. Fitz denies there's anything between them, but Chen doesn't believe it and demands to know Fitz has returned to Toronto.

Five years earlier, Ming suffered a miscarriage. Back in the present, Fitz goes to see Ming to discuss their insemination plans. While he offers her the option to bow out, it's apparent he's the one who is feeling ambivalent.

In a flashback to Christmas Eve three years earlier, the complicated and interlinked early relationships between Ming, Chen and Fitz come under the spotlight.

Fitz is sound asleep and hung over when he gets a call to go to the airport for an emergency transport from Guatemala. At the hospital, Ming is with Janice who is in pre-labor. Meanwhile, Chen gets roped in for a poker game.

Flashback to years earlier when Ming and Chen are thinking of adopting. Fitz comes down with a serious illness.
