Surgical resident Dr. Grace Devlin (Jordana Spiro) juggles her promising medical career with a secret deal to offer her medical services to a mob boss in order to save her ne'er-do-well younger brother's life.

With Paul Moretti out of the picture, Constantine Alexander (William Forsythe) calls on Grace (Jordana Spiro) to treat the fugitive mobster he's expecting to partner with in a shady video poker deal. Meanwhile, Grace and Brett (Zach Gilford) treat an ailing bride who's bolted for the emergency room in the middle of her wedding.

After treating a family that was involved in a fatal car accident, Grace (Jordana Spiro) is stunned when Constantine (William Forsythe) orders her to treat the hit-and-run driver's injuries, too.

After delivering a baby that Constantine (William Forsythe) is using to blackmail a politician, Grace (Jordana Spiro) tries to help the mother keep her newborn son. As Grace's treatment of an old friend uncovers a dangerous brain malady, Moretti (Michael Rapaport) looks to stop a witness from testifying at his trial.
