On the heels of her and Tom's (Michael Vartan) wedding, an attack in the hospital parking lot endangers both Christina (Jada Pinkett Smith) and her unborn child. And as Morrissey (James Morrison) looks to appoint a new head nurse, Steve (Adam Rayner) makes an important decision about his future.

While recovering from her attack and struggling to cope with being replaced by Bobbie (Suleka Mathew), Christina (Jada Pinkett Smith) tries to protect an abandoned girl from being put into foster care. Meanwhile, the frustration with his inability to protect his new wife leads Tom (Michael Vartan) to take drastic measures.

Christina (Jada Pinkett Smith) is drawn back to the hospital when Nick (Mark Anthony) asks for help in treating his headstrong mother (guest star Miriam Colon). And when Harry Foxe (guest star Ben Cross) arrives for weight loss surgery, Steve (Adam Rayner) must acknowledge that the English crime boss is his estranged dad.

As the Internal Affairs investigation into the death of Christina's (Jada Pinkett Smith) attacker strains her relationship with Tom (Michael Vartan), Kelly (Vanessa Lengies) can't get anyone to listen when she suspects that a dangerous infection is spreading through the hospital. Meanwhile, Camille (Hannah Hodson) plunges headlong into Miles's (Derek Luke) messy personal life.

As she and Tom (Michael Vartan) look to repair their fractured relationship, Christina (Jada Pinkett Smith) is surprised by Camille's (Hannah Hodson) decision to stay in Richmond for now. As Camille helps Miles counsel a cancer patient, Nick (Marc Anthony) undergoes a harsh interrogation about his role in the murder of Christina's assailant.

As Christina (Jada Pinkett Smith) persuades Nick (Marc Anthony) to have a birthday party for his ailing mother, Camille's relationship with Miles (Derek Luke) is endangered after her negligence nearly kills his son. Meanwhile, Gail (Vanessa Bell Calloway) confronts Morrissey (James Morrison) about his drinking.

With Morrissey in rehab, the no-nonsense board member brought in to serve as interim director has some surprises in store for Christina (Jada Pinkett Smith) and Bobbie (Suleka Mathew). And as Bobbie tries to enhance her reputation by enlisting a smooth-talking investment manager to help with fundraising, Christina's strong feelings for Nick (Marc Anthony) threaten to upend her marriage.

As the problems brought on by her affair with Nick send Christina (Jada Pinkett Smith) to see a therapist, Tom (Michael Vartan) makes it clear that he won't stay on at James River if his wife is hired to run the hospital. Meanwhile, Steve (Adam Rayner) struggles to help an EMT who's been diagnosed with ALS.

As Christina (Jada Pinkett Smith) looks to move beyond her divorce, her plans to leave town are upended by the death of Nick's (Marc Anthony) mother. And as Bobbie (Suleka Mathew) learns that the money she lost to a con man may be returned, Steve (Adam Rayner) struggles with his estranged father's grave illness.

As EMT Tony Gee (guest star Cress Williams) refuses treatment for an injury so that he won't have to live with ALS, Christina (Jade Pinkett Smith) returns to the hospital in the wake of her divorce even as she worries about Nick's (Marc Anthony) sudden disappearance.
