In Somerset, District Nurse Shonna drives hundreds of miles a week through the countryside, looking after people in their own homes. Her patients include a decorated war veteran, and a former tea-lady who retired at 80, but is determined not to be defeated by her health problems.

In rural Yorkshire, Midwife Kay is part of a team who help mums have home births, often miles from the nearest hospital. But babies aren't born to a timetable, so she carries a mobile phone, which can go off any time of day or night. This week, she is woken at 3am by a call from Natalie, who is at home with her husband and has suddenly gone into labour.
