As the drug lord is prepared for surgery, a man bursts in and disconnects him from the machines, killing him. Santiago wants to quit the practice. The Butcher attacks Julia and kills Jessica. Meanwhile, an elderly woman wants the doctors to make her look younger so her husband with Alzheimer's disease will remember who she is. Also, Cristóbal finds out that Kimberly is working for his former partner, Mauricio, who opened his own clinic. And after Carla almost has a miscarriage, Julia calms her fears about Cristóbal.

When Glorita again demands free cosmetic surgery, she gains Santiago’s sympathy by claiming that her daughter has cancer. But her daughter reveals that Glorita lied, which has harmful effects. Meanwhile, Cristóbal helps Kimberly after Mauricio's breast augmentation procedure damages her skin, and he persuades her to report Mauricio to the authorities. Cristóbal vows to adopt Carla's baby as Carla's promiscuous behavior becomes more perverse. And Alicia's boyfriend, Felipe, cannot control his jealousy of her friendship with Santiago.

Susana breaks up with Matías, prompting him to get arrested and attempt suicide. Meanwhile, when another of the Butcher's targets arrives at the clinic, Cristóbal begs Santiago to stop provoking the Butcher by operating on his victims. But Quintero volunteers to help. Santiago and Cristóbal have a heartfelt discussion about Julia. And after Alicia's boyfriend hits her, Santiago beats him up.

But it's not Santiago's house that the Butcher visits—it's Cristóbal's.

Cristóbal is traumatized and unable to work after the Butcher's attack. Meanwhile, student Carolina invites Matías to a prayer group, not knowing that he is the one who hit her with his car. Santiago begins to make his feelings for Alicia clear, unaware of her attraction to Cristóbal. And Santiago's scheme to persuade Cristóbal to return to the clinic and help deal with Quintero works.

Alicia sleeps with Santiago, her best friend's ex-husband. Quintero questions Julia about her relationship with Cristóbal. Meanwhile, Alicia finds out she has a serious illness. And Angelica returns for a spa visit, making Julia tense. Later, Julia and Matías find out that Angelica’s airplane leaving town crashed with no survivors.

Upset over his grandmother's death, Matías continues abusing pills. Later, he is shocked to see Santiago having sex with Alicia. Santiago also learns about Alicia's advanced lymphoma. Meanwhile, Mari, a girl who dated Matías, finds out she is pregnant and tries to induce an abortion with pills. And while operating on a transvestite's scars, Santiago offers to do a sex change operation as well.

Possessing evidence implicating Johanna as the Butcher, the police arrest her. Later, they set their sights on Quintero—until the Butcher strikes while Quintero is in custody. Cristóbal considers adopting his and Carla's baby. While Julia comforts the ill Alicia, Santiago accidentally reveals his affair with Alicia. And Matías tries to prevent his pregnant former girlfriend from being expelled.

The Butcher's latest victim is Elena, the clinic's lawyer and Johanna's friend. Meanwhile, Santiago and Alicia offer to adopt Matías and Mari's baby. Mari is amenable, but Matías wants to make a family with her. And on the day of her wedding to Cristóbal, Kimberly wonders if he is still in love with Julia.

Cristóbal is in shock after being abandoned by Kimberly on their wedding day. But after Kimberly is missing for more than two days, everyone suspects there's more to her disappearance. The Butcher mutilates her face to counteract the plastic surgery she’s had and mails her extracted breast implants to Cristóbal. Kimberly is later found wandering the city in shock.

Glorita begs Cristóbal to do one last surgery on her. Kimberly remembers nothing about her ordeal with the Butcher, but Quintero remains the main suspect. The Butcher attacks several porn stars where Kimberly worked. But Quintero is found too—mutilated and crucified. Quintero persuades Santiago and Cristóbal to operate on him at the clinic—where, as the Butcher, he carries out his final revenge.

After Quintero, a.k.a. the Butcher, takes Cristóbal and Santiago hostage, the police chief helps Quintero escape! Kimberly finally lures Quintero out of hiding and empties a gun into him. Meanwhile, Alicia takes steps to spare herself and others from further pain. Mari and Matías must give their child to adoptive parents. And Santiago quits the plastic surgery practice forever, for a new life.
