Six WWF stars, Matt Hardy, Jeff Hardy, Test, Mighty Molly, Lita and Jacqueline, compete to win a $50,000 grand prize for charity. They must climb a rope ladder beneath a helicopter, drink a shake with pig brains and transfer flags across telephone poles.

Twins team up to collect flags from a platform suspended 100 feet in the air. Then, they compete alone in a game of billiards to avoid eating live ants, habanero peppers, shiokara and century eggs. Finally, they must leap between shipping containers.

Contestants must leap from rooftop to rooftop 10 stories high, retrieve and eat balls of coagulated blood hidden in a plate of worms and escape from an airplane fuselage submerged in water.

Thirteen players who have won before on Fear Factor during the second season compete again for the Fear Factor Championship and a $100,000 prize.

The remaining nine players battle it out in the final stunts and compete for a grand prize of $100,000.
